If a member of staff contacts you saying that they think you would benefit from a clerical recheck or a review of marking, then you must complete this form giving the school permission to do this.

The outcome of one of these procedures may be that:

* Your original grade may be lowered
* Your original grade may remain the same
* Your original grade may be higher

We will not request this appeal until we have your written permission in the form of this completed form.
